Activity rate between 60% and 100% Permanent contract

Your mission Assess the client’s needs from a socio-economic perspective as well as their level of independence for carrying out administrative activities while relying on available resources Restore conditions that allow clients, their relatives, or a third party to manage administrative activities independently Assert clients’ rights with insurance and social assistance Actively contribute to clients’ home care through specific measures while considering individual needs with personalised care and promoting autonomy Facilitate procedures for day care or temporary or long-term accommodation Encourage and support clients and their relatives by maintaining their autonomy and quality of life as much as possible according to their abilities Actively collaborate in interdisciplinary team activities with a distinct role allowing you to value your profession as a social worker as a full participant in the healthcare system

Your profile and skills Bachelor’s degree in social work with a focus on social services (or SECO recognition) Proven experience of 2 years in the medico-social field Strong interest in community health Ability to collaborate in networks and multidisciplinary teams Sense of responsibility, ability to organise autonomously and set priorities
