You will work within a motivated and caring team in an environment where human contacts are at the heart of the activity.In order to complete our team in Renens Sud, we are looking for, immediately or by agreement : Health Promotion Nurse (100%)As a Health Promotion Nurse, you will carry out prevention and health promotion actions for clients with health concerns, in order to promote their long-term maintenance at home : - Ensure the assessment and follow-up of care- Provide follow-up with the client, their surroundings, the multidisciplinary team, as well as with partners- Guarantee an updated client dossier- Conduct screening of vulnerability elements and risk factors among clients- Identify resources of the client and their surroundings and strengthen their capacity to act- Implement actions to ensure home maintenance according to the quality of life desired by the client- Strengthen the client’s ties with their formal and informal network and support them in their démarchesProfile and skills : - Degree in nursing (HES) or a recognized nursing title in Switzerland- At least 2 years of experience in the field of care- Good clinical analysis and synthesis abilities- Comfort in task management and in management tools for prioritization and decision-making- Proven experience in community care with public health actions- Ability to work within an internal and external network- Knowledge of the Vaud care network preferred- Proficiency in the French language with good writing skillsWe offer : - Working conditions according to the Collective Labor Agreement (CCT) in the para-public health sector in Vaud- Professional insurance conditions that extend beyond the minimums provided by law- Five weeks of vacation per year and six weeks beginning at age 50- Rich and varied offerings related to the position heldWorkplace : CMS X NordStart date : Immediately or to be agreedWant to join our team?
